License numbers starting with the letter "T" are Teaching Permits.
License numbers starting with the letter "V" are Volunteer Licenses.
Fill in the desired information below and select the Search option. All of the fields are optional, but at least one of them must be filled in. The more fields that are filled out, the more specific the results returned will be. The Search can be in partial using the wild card (*). To use a wild card search enter an * at the beginning or end of your search. For Example, if you are searching for someone with the last name of Johnson, you can use the full name or as John* to help return a wider range.
If you would like to look up a Corporation or PLLC, please enter their name in the "Business Name" field and/or their License Number in the "License Number" field, but maintain License Type as "All".

For Licensees
To obtain a written verification of licensure to be sent to another state, a written request must be submitted along with a fee of $23.00 per verification. The request must include the following information:
- Licensee’s Name
- Telephone Number
- Name of State the Verification is for
- Where the verification is to be mailed
For Other Entities
To obtain a written verification of a dentist or dental hygienists licensure status, a written request must be submitted along with a fee of $23.00 per name. Please supply as much information as necessary to provide the correct verification.
